Most likely causes of thread breakage
Damaged needle
Damage to inside edge of presser foot or needle hole
Damaged sewing hook
Incorrect sewing hook to needle distance
Incorrect sewing hook to needle timing
Poor quality embroidery thread
Incorrect thread tension adjustment
Excessive stitch density in design
Excessive numbers of very small stitches in design
Needle size is too thick for fabric
Incorrect presser foot height adjustment

Step-by-step-fault finding
The secret is always to start with the simplest and most obvious possible causes.
Does it happen equally on all needles or just on one needle?
If it's just on one needle then the problem is above the needle plate.
Change the cone of thread
Check for correct threading including the check spring
Check tension adjustment
Change the needle
Check for damage to inside of presser foot hole
If it happens on all needles on one head only
Check for damage to the inside of the needle hole
Check for damage to the sewing hook
Check hook to needle distance
Check sewing hook timing
If it happens randomly on all heads and on all needles.

Check design for excessive stitch density and excessive small stitches
Try another design which you know has worked well before
